ENROLLMENT PROCEDURES

Requirements

• Entry Requirements

            * can communicate both in oral and written;

            * physically and mentally fit (must submit health certificate)

            * with good moral character

            * can perform basic mathematical computation

• Enrollment Procedures

            > 1st Step (For Taguig Enrollees Only; Pasay Enrollees may proceed to the 2nd Step):

                        Take the Profiling Exam (Schedule: Mon-Thurs, 8am or 1pm, proceed to TESDA Gate 1)

            > 2nd Step:

                        Proceed to CATFABS office 
                                    Taguig: Bldg. 5, TESDA Complex, East Service Road Taguig City 
                                    Pasay: 2nd floor Isabel Bldg. Taft AVenue, Pasay City
                        and submit the following:

                        1. Submit the following complete documents:

                                    - Resume

                                    - 2 pcs. 2x2 picture

                                    - NSO Birth Certificate (photocopy only)

                                    - NBI or Brgy. Clearance (photocopy only)

                            - High School / College / Technological Diploma  / Transcript of Records (photocopy only)

                                    - Medical Results (you just be cleared from Tuberculosis, Hepatitis, Asthma, Heart Ailment and Drug Test)
                                      (photocopy only)
Accredited Clinic Referral Slip and Site Map:
 

                        2. Completely fill-up registration forms.

                        3. Pay at least fifty percent (50%) down payment to reserve a slot. 

• Uniform Requirements: P1,000.00 starting August 1, 2012

            ♦ CATFABS T-shirt

            ♦ Chef's Coat

            ♦ Chef's Hat

            ♦ Apron

• Additional Uniform and Grooming Requirements (must provided  and observed by trainees)

            ♦ Black Pants (Slacks),  Jeans or Maong not allowed

            ♦ Black Leather Shoes  (Rubber shoes not allowed) 

            ♦ Side Towel

           ♦ Gentlemen must wear proper hair cut, must be well-shaven and no jewelries are allowed.

            ♦ Ladies must wear hair properly, must be well-tied or may use hairnet if necessary.

            ♦ All trainees must refrain from wearing jewelries during training.
